This notice describes the current ClearHarbor Health website, for health coverage inquiries.

Information you choose to provide

The request form asks for your name, email, phone, age, ZIP code, coverage interests, preferred contact time, and consent choices. Requests are submitted directly to our shared ClearHarbor request service hosted at clearharbormedicare.com and saved in its hosted database with the submission time and exact consent wording. No email app is required.

How requests and preferences are used

Details you send are used to handle your inquiry. Optional email and text marketing choices are saved separately with your request. If you opt into email marketing, we may send your name, email address, and consent details through Zapier to Constant Contact to manage email communications. Age, coverage interests, and phone number are not sent through this email marketing connection. We do not automatically enroll you in text marketing. Declining optional marketing does not prevent a coverage inquiry.

Website services

The hosting and security services may process technical information such as IP addresses, browser details, request logs, and security cookies to deliver and protect the site. Fonts may be requested from Google Fonts. Resend processes submitted request details to send notification emails to our team.

Your choices and questions

Do not submit medical records, Social Security numbers, or payment information through this form. To ask about information you have sent, request correction or deletion, or withdraw a contact preference, email info@financialfreedomhub.net or call (917) 663-6562.
